To make your visit as smooth as possible, please review the following:
Arrival Time
New patients: arrive 20–30 min early, or complete forms online at gilbertcenter.net/new-patient-start-here. Established patients: arrive 10–15 min early.
Location & Parking
652 E. Warner Road, Suite 107, Gilbert, AZ 85296. Free parking is available in the lot adjacent to the building.
Cancellation Policy
Cancel or reschedule at least 24 hours ahead by calling 480-539-8680. Late cancellations and no-shows may result in a fee.
Patient Portal & Telehealth
Access the Patient Portal to view lab results, request prescription refills, send messages to your care team, and pay your bill online. Telehealth appointments are also available.

New Patient Checklist
Please bring the following items to your first appointment:
- Photo ID — driver's license, state ID, or passport
- Current Insurance Card
- Copayment & Deductible payment method, if applicable
- Payment Method to be saved on file
- All Medications — prescriptions, supplements, vitamins, and OTC medicines (bring bottles or a written list)
- Living Will and/or Power of Attorney, if applicable
Annual Visit Checklist
At each annual wellness visit, please bring the following updated items:
- Updated Photo ID, if it has changed or expired since your last visit
- Updated Insurance Card — we verify coverage at each visit, even if unchanged
- Current Medication List — any changes since your last visit
- Living Will and/or Power of Attorney, if there have been changes

Your annual wellness visit is also a great opportunity to:
- Review preventive screenings and immunizations
- Update emergency contacts and pharmacy information
- Set health goals with your provider for the coming year
