2. No doctor-patient relationship

Use of this Site, including submitting a contact form, requesting an appointment, or exchanging messages through the patient portal, does not by itself create a doctor-patient relationship. A doctor-patient relationship is established only after you have been formally accepted as a patient and have had an in-person or telehealth encounter with one of our providers, consistent with our internal registration and intake procedures.

The content on this Site — including text, graphics, images, and other material — is for general informational purposes only and is not intended as, and should not be relied upon as, medical advice, diagnosis, or treatment. 4. Appointments and cancellations

Requesting an appointment through the Site does not guarantee a confirmed appointment time; all requests are subject to confirmation by our staff. 5. Patient portal

If you are issued credentials to access our patient portal, you are responsible for maintaining the confidentiality of your login information and for all activity that occurs under your account.
