As the providers’ trusted right hand, this dedicated team manages the flow of clinical operations, helping appointments stay on schedule while supporting patient care every step of the way. They assist with patient rooming, care coordination, results reporting, medication and treatment questions, and communication between patients and providers.

Chronic Care Management (CCM)
Our Chronic Care Management (CCM) Team is dedicated to helping Medicare patients stay healthy, informed, and connected to their care between office visits. With more than 40 years of combined experience as medical assistants, these compassionate professionals serve as a trusted point of contact for patients managing chronic health conditions.
Each patient is paired with a CCM team member who provides personalized support, helping coordinate care, answer questions, monitor health concerns, and ensure medications are taken safely and effectively. By maintaining regular communication with patients and providers, they help address issues early, promote better health outcomes, and make navigating healthcare less overwhelming.
